Work Permits
Work permits are processed through the
SHS Counseling Office
SHS Students:
- Get an Application for Employment Certificate from the Counseling Office or print one from the link below
- Fill out part 1 and have parent/guardian sign it
- Go to the nurse - they will complete part 3 (certificate of physical fitness - physical within 12 months verification)
- Then bring the application to the Counseling Office where they will complete part 2 (evidence of age) and issue the permit card
- You must sign the permit card in front of a Counseling Office staff member
All Other Students (Pittsford residents who are home schooled or attend private schools):
- Get an Application for Employment Certificate from the Counseling Office or print one from the link below
- Fill out part 1 and have parent/guardian sign it
- Return the application to the Counseling Office. You must provide proof of age and identity (Passport or birth certificate) and proof of having had a physical within the past 12 months.
- A Counseling Office staff member will process a card for you that you must sign in front of the staff member
Please Note:
- The employer will keep the employment certificate until the employee leaves or turns 18.
- If you lose your employment certificate, the Counseling Office can create a duplicate for you.
- If you have a work permit card for ages 14 - 15 (blue), you can get a work permit card for ages 16 - 17 (green) from the Counseling Office when you turn 16. You do not need to do the paperwork again.
- Child model and farm worker employment certificates are also available.
